Role Overview:
As a Home Health Registered Nurse (RN) at Elite Healthcare, you will play a crucial role in providing compassionate and comprehensive care to patients in their homes. Working closely with attending physicians, patients, families, and other members of the patient care team, you will assess patient needs, collaborate on care plans, and help patients regain their independence. Your dedication to excellence and leadership qualities are essential as you actively participate in interdisciplinary team conferences and ensure continuity of care.
Responsibilities:
- Assess and reassess all physical, environmental, and emotional factors to determine home health needs for patients.
- Collaborate with patients, families, and the interdisciplinary team to address emotional needs and promote patient well-being.
- Actively participate in Interdisciplinary Team (IDT) conferences to coordinate and optimize patient care.
Requirements:
- Positive and friendly attitude with a passion for delivering exceptional care.
- Current and unencumbered license to practice as a Registered Nurse in Utah.
- Current CPR certification.
- Valid driver's license with reliable transportation and agency-required liability insurance.
- Open-mindedness and willingness to learn and grow in the role.
- Must pass a background check.
