Job Title: RN Case Manager – Community-Focused Nursing Role
Company: Home Health Care, Inc.
Location: Southwest Minnesota Region
Schedule: Full-Time (M-F, 8:00 AM – 5:00 PM), 1 weekend/month, 1 on-call shift/week
Compensation: $85,000 – $130,000 annually + $8,000 in bonuses during the first year
