Job Description

Responsibilities:

• Manages referral, admission and discharge processes for program
• Develops individual Recovery Plans(IRPs) with consumers and completes additional required documentation
• Supports and assists consumers with case management duties and crisis intervention as needed
• Engages family involvement and act as a liaison between the program and other providers such as local hospitals
• Facilitates consumer appointments with APN, court system or outsider providers as needed
• Participates in 24-hour Clinical On-call rotation to provide support to staff after hours
• Provides orientation training to new employees and ongoing clinical support and direction to direct care staff

Qualifications:

• MSW in Social Work, MA in Psychology, or a related field
• Previous experience in mental health / human services
• LSW or LAC preferred
• Valid New Jersey driver's license

Shift:

Monday through Friday, 9am-5pm
