Social Service Worker Location Pittsburgh, PA : Corner View Nursing and Rehab is seeking dedicated, caring, and personable Social Services Worker to be part of our team! We are a 187 bed skilled nursing home located in Pittsburgh Our mission is to provide compassionate and quality care while meeting the physical, social, and emotion needs of the residents. Job Responsibilities: Social Services Worker functions in conjunction with the Social Services Director as listed below:

• Complete portion of MDS for residents

• Ensure resident care is being performed in a safe manner

• Develop discharge plans that are appropriate to the residents needs

• Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to Administrator and QI Committee

• Assist in the design of a social service program that meets State, Federal, and Division guidelines

• Delegate authority, responsibility, and accountability to other responsible department personnel

• Participate in Quality Assurance programs, and any other facility committee, or program which seeks to improve the performance and accuracy of resident care.

• Communicate with residents, family members, staff, and management team

• Ensure that care provided is in accordance with the resident and family wishes

• Other duties as assigned

Qualifications: BSW or a degree in Gerontology. 1 year long term care experience preferred. Job Type: Full-time
